We Provide
Emergency Assistance

Sunnyside is linked electronically through the Emergency Assistance Network with the Forsyth Department of Social Services, Crisis Control Ministry, the Salvation Army, the Experiment in Self Reliance, and other agencies. In order for the network to function as it should, basic requirements are necessary so that these agencies may better coordinate service to those in need. When referrals to other agencies are necessary, this information sharing enables the need of the client to be met in a more effective and efficient manner.

The process of determining need and our assistance includes application, interview, and verification.

Items needs for the process of application and interview include:
  • Photo Identification
  • Social Security number in printed form
  • Proof of current address (printed on a bill or a piece of mail)
  • Proof of any income for the last 30 days
  • Proof of expenses for the last 30 days (receipts, bank statement)
Sunnyside must see a picture ID, a Social Security or ITIN card and proof of residence in our service area. Proof of the crisis such as utility cut off notice or eviction notice is needed.
Also required are proof of family income and receipts for how that income has been spent in the past thirty days. Other verified information may be necessary in order for Sunnyside to make an informed decision about the level of assistance needed.
