In collaboration with the Forsyth County Department of Public Health Sunnyside provides a neighborhood health clinic on the first and third Thursdays of each month. The clinic is open to anyone who lives in Forsyth County.

• The first Thursday is for children and adults.
• The third Thursday is for children only (under age 22)
• We do school physicals, but not sports physicals.
• Our doctors give regular checkups and treat illness.
• Routine shots and lab work are provided.

Intake begins at 5:30 p.m. at Trinity Moravian Church. Trinity Moravian Church is located on the corner of Sprague Street and Lomond Street. The address is 220 E. Sprague Street, Winston-Salem, NC 27127.
